How Do I....Register With The Practice? Please bring with you details of your current doctor and also your NHS number if you know it. You will be required to complete a registration form which will allow us to register you with the relevant Health Authority and provide us with photo ID and proof of address e.g. recent utility bill. All newly registered patients will be asked to complete a simple questionnaire and invited to attend the practice for a new patient assessment. This is important as it often takes up to three months or longer for your medical records to arrive. It is important to arrange for your assessment as soon as possible so any prescriptions can be placed on the computer. Please note you are not obliged to see the doctor with whom you are registered. It is practice policy to allow patients to choose whichever doctor they wish to see in the partnership and for continuity it is best to stay with the same one. Please bring an adult interpreter with you if you have difficulties understanding what is being said. We operate a zero tolerance attitude to verbal or physical abuse by patients. If any patients are believed to be infringing this rule, Hailey view surgery reserves the right to remove patients and their family from our list.
